Columbiana Health and Rehabilitation, LLC was the third largest nursing home in Shelby County during the first quarter of 2026, Centers for Medicare and Medicaid Services (CMS) data shows.
The nursing home offered 63 beds, which was 68 fewer than the county’s average for the quarter, and cared for an average of 61.1 residents per day.
Columbiana Health and Rehabilitation, LLC received an overall CMS rating of 4 out of 5, surpassing Alabama’s nursing home average rating of 3.
For the same period, The Healthcare Center at Buck Creek was Shelby County’s largest facility with 198 beds, followed by Shelby Ridge Nursing Home, which had 131 beds.
As of 2026’s first quarter, Columbiana Health and Rehabilitation, LLC had incurred $8,018 in fines and had received one penalty.
Three for-profit nursing homes operated in Shelby County, Alabama, during the period.
U.S. Census Bureau estimates indicate the nation’s older adult population is increasing as numbers in younger age groups decline. Americans age 65 and older grew by 13% from 2020 to 2024, while those under 18 declined by 1.7%. The country’s older population has expanded significantly over the last 100 years, from 4.9 million in 1920 to more than 61 million by 2024.
A 2021 study estimates that almost half of those 65 and older—about 28 million people—may need long-term services and support. The study also predicts that over one-third may eventually require nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
